I think its what you are used to but what is comfortable for the patients example PAV+ is supposed to be better than PSV, I use Bilevel only as a main mode because it provides everything you need. I also use use PAV+, PSV, & TC

This question and others like it always interests me. I don't think there is a "best" mode of ventilation for all patients with any diagnosis. I think there are to many factors to simply put out a best. I also don't think that all vents are created equal. There are to many factors just with vents, not to mention RT experience and understanding of each and every mode in all of the vents available. There are some significant differences in modes simply based on the technology of the equipment and how the specific manufacturer chooses to implement it. Monitoring packages and exhalation valves also influence modes to a point, so I think the question is to broad to attempt to answer. I also think that there needs to be some division in vents that we are losing with the one size fits all type of vent. I don't like the idea of one vent is the best for all ages, sizes, diagnosis and situations. To many functions and variables with a single piece of equipment makes it harder to learn, and harder to understand the subtle differences that are associated with some equipment. That being said there should be a separation of vents based on the patients condition. Some vents are great for ARDS, and other conditions where it is difficult to ventilate a patient, where as they are not likely to be as patient friendly if respiratory failure is not the reason they are on a ventilator. I will get off my soap box now and simply end by saying all the smoke and mirrors that some manufacturers are using to sell there new modes of ventilation are just that. I haven't seen a new mode of ventilation in more than 10 years, and I don't plan on seeing one any time soon. They are all just twists on old modes using new names to make them sound good. Yeah the electronics has gotten better, but they are still just the same old mode...

in what case ur asking it depends on the diagnosis and severity of the disease and infection the patient have( for ex: ARDS,COPD)
