Jeff Whitnack replied to Paolo's discussion 'NIV/ BiPAP vision machine on a tracheostomized Pt.'
You can do it. I just consider it an act of desperation and not inspiration. The main proposed "use" for it at my hospital is so they can transfer trached patients who DO need mechanical ventilation to floor wards where "Bipap"…

Jeff Whitnack replied to Johani's discussion 'Which mode is better, A/C or SIMV?'
I call SIMV "Stupid Idiot Mode Ventilation". I think it initially arose as 1) early A/C was very patient unfriendly as regards spontaneous breathing---lousy trigger, lousy flow response, tethered to piston setup. Early A/C had trigger…

Jeff Whitnack replied to Anwar Nadeem's discussion 'better method for weaning either T-Piece or PS'
I don't like the use of the phrase "weaning" when describing our efforts to get patients off the ventilator. I also don't like the term "liberate", although it describes the end result it still seems to imply that our…

Jeff Whitnack replied to Paolo's discussion 'NIV/ BiPAP vision machine on a tracheostomized Pt.'
I think using a Vision Bipap (or new V60) on a floor trache patient should be viewed as an act of desperation, not an act of inspiration. (inspire as in "the teacher inspired his students to.." and not in the air exchange manner, heh…

Jeff Whitnack replied to Tanveer Abbas Khan's discussion 'RT &RN'
I have been an RT since 1982. Looking back some of my finer moments have occurred when working well together with RN(s). And some of my most infuriating/frustrating moments have occurred when working not so well with RN's. To a degree the same…

Hi Prem, What exactly are you looking for as regards humidification? I disseminated a study a while back by Brochard and others in which they showed that active humidification isn't all it's supposedly touted to be (heats more than…

Hi Jeff
Yep its like a madhouse here also I will like a kangaroo leaping from one situation to another and also we are in the process of closing down the other hospital i cover and on Friday 13th March we are transferring all the ICU patients from that hospital over to Watford where I am so ive got the big task in making sure everything is in place and its safe. The head nurse is a bit of a nutter but i'm coping with that. Anyway its the weekend and time to relax.

All The Best
