should I wean a chronically ill patient whose weaning parameter show he can be weaned. I had a patient with CA Esophagus ( Status post esophagectomy 24 hrs. At the end of the shift his HR was 145. but his ABg and rest of the Respiratory finding were normal. I took the chance to extubate him. 8 hours later he went on bipap. next day he was completely off. He was very happy and appreicative of the fact that I extubated him. This was the only chance he had I took it. What do you all think.
